Number Sense Challenge registration form

Each student who participates in the Number Sense Challenge must be supervised throughout the event by an adult chaperone, whose email address must be filled in below. A chaperone can be a parent, teacher, math coach, or any other responsible adult who agrees to serve as chaperone according to the rules below. The chaperone does not need to be the school's sponsor of record.

When registering for the Number Sense Challenge, each student must determine what grade level and school they will be in on the last day they attend school prior to the last test of the Challenge. The student must list this grade level and school on their registration form. In particular, for the Challenge that runs from July to September each year, students will most likely report the grade level and school they will be in for the upcoming school year.

By registering for this contest, you agree to the following:

You have read and understood all mathleague.org policies and procedures associated with this contest, including but not limited to our Conduct Standards, and you will comply with the appropriate rules and do your best to ensure that the students under your care do so as well.
In particular, you acknowledge and agree to our policy regarding release of results and photographs: "Any student's name, grade, school, and scores (and photos, if any are taken) could potentially be reproduced on the mathleague.org website and/or through other media. Any schools or individuals who wish to avoid such publicity must contact mathleague.org prior to the start of the contest for which they wish to opt out of such publicity."
You and any students whose email addresses you give us will be placed on our mailing list, but you can unsubscribe at any time after receiving your first email if you'd rather not receive information about future contests.
Number Sense Challenge registrations are nonrefundable after the Challenge starts. If you wish to request a refund prior to the beginning of the Challenge, please contact us and we can process your refund minus a 10% processing fee.
