mathleague.org membership form
By submitting your entry below on behalf of your School, you affirm that either (1) you are the person authorized by the School to serve as the School's math team sponsor for mathleague.org events or (2) you are an adult over the age of 21, you have made an effort to secure such authorization from the School but the School has declined to authorize any person as the School's math team sponsor for mathleague.org events, and you agree to accept the responsibility of serving as the School's math team sponsor for mathleague.org events. You also affirm that you have read all the rules and regulations found at http://mathleague.org pertaining to any contests in which your school will participate, and that you will abide by them as a sponsor. You further certify that the information you submit is complete and accurate to the best of your knowledge, and that you give mathleague.org permission to add your email address to its internal email distribution lists so that mathleague.org can communicate important information relevant to your membership.
Be sure to fill out as much information as you can, and to use a valid email address. You are given an opportunity to create a password that will allow you to access your membership information and to make any necessary changes once this form has been submitted; please choose a password that you do not use for any other purpose, as mathleague.org does not assume responsibility for any consequences that may arise as a result of your password being intercepted.
