The mathleague.org Membership Form Please do not fill out this form unless you have checked our membership roster to verify that your School has not already filled out a membership form for this year. By submitting your entry below on behalf of your School, you affirm that you are eligible to serve as the sponsor for your School as defined below and that you accept the responsibilities of being a sponsor as enumerated below. For schools in the US: A sponsor must be either (1) the person authorized by the School to serve as the School's math team sponsor for mathleague.org events or (2) an adult over the age of 21 who has made an effort to secure such authorization from the School but the School has declined to authorize any person as the School's math team sponsor for mathleague.org events. A sponsor must not exclude or discourage any student from participating in a contest for which the student is eligible, must not withhold from any student any relevant information regarding any contest the student registers for, and must not withhold from any student access to his or her completed test papers after each contest is over. A sponsor must read all the rules and regulations found at https://mathleague.org pertaining to any contests in which the School will participate (including verifying that the School fits the definition of a school at least for the students the School will be enrolling in our contests), and make every effort to ensure all participants from the School in our programs understand and abide by these rules. As a sponsor, you certify that the information you submit below is complete and accurate to the best of your knowledge, and that you give mathleague.org permission to (1) share your name and email address with other participants from your school and (2) add your email address to its internal email distribution lists so that mathleague.org can communicate important information relevant to your membership. As a sponsor you may request from us the email addresses and registration information for students from your school who are registered for our contests. Be sure to fill out as much information as you can, and to use a valid email address. You are given an opportunity to create a password that will allow you to access your membership information and to make any necessary changes once this form has been submitted; please choose a password that you do not use for any other purpose, as mathleague.org does not assume responsibility for any consequences that may arise as a result of your password being intercepted. Once you have submitted this form, it will be sent to our team for approval. As long as you have not made any mistakes in filling out the form, your membership should be approved within 24 hours.
