Relay Challenge registration form

Each K-12 student who participates in the Relay Challenge must be supervised throughout the event by an adult chaperone, whose email address must be filled in below. A chaperone can be a parent, teacher, math coach, or any other responsible adult who agrees to serve as chaperone according to the rules below. The chaperone does not need to be the school's sponsor of record.

By registering for this contest, you agree to the following:

  • You have read and understood all mathleague.org policies and procedures associated with this contest, including but not limited to our Conduct Standards, and you will comply with the appropriate rules and do your best to ensure that the students under your care do so as well.
  • In particular, you acknowledge and agree to our policy regarding release of results and photographs: "Any student's name, grade, school, and scores (and photos, if any are taken) could potentially be reproduced on the mathleague.org website and/or through other media. Any schools or individuals who wish to avoid such publicity must contact mathleague.org prior to the start of the contest for which they wish to opt out of such publicity."
  • You and any students whose email addresses you give us will be placed on our mailing list, but you can unsubscribe at any time after receiving your first email if you'd rather not receive information about future contests.

Please submit your registration for this year's Relay Challenge below. DO NOT include any apostrophes anywhere in the form or it will generate an error! If you are an adult who is planning to compete in the open division, please indicate what school you are affiliated with (eg. if you are a teacher at a school or a parent with a student in a school); you may put NO SCHOOL for your school name if there is not a school you are affiliated with. All K-12 students planning to compete must indicate the correct name of the school they actually attend.
